For [photocopy requests](http://confluence.soutron.net/Requests_and_reservations.html#Photocopy_requests), the user has to print a copy of the Copyright Declaration Form and sign it, and library staff need to confirm receipt of this before a photocopy request can be actioned.

The form is generated from a file called <span style="color: #008000">CopyrightDeclarationForm.pdf</span>, which is located in the Library folder of the application. A sample form is supplied with the system:

# Creating your own copyright declaration form

To replace the sample form with your own:

1. Create your own form in Word or similar package.
2. Turn it into PDF format.
3. Name the file CopyrightDeclarationForm.pdf
4. Copy it into the Library folder of the application, replacing the sample one which is there.
